On the day of your appointment, you will need the following:

Photo identification
Insurance card
Any prescription(s) from your doctor’s office with diagnosis
Form of payment for co-payment or co-insurance, if applicable

Financial Arrangements

After providing us with accurate and current information, our billing department will bill your primary insurance carrier. However, you are ultimately responsible for the bill for services you receive. You will receive a separate bill from the radiologist for his or her professional fee, which covers the cost of reading your study and interpreting the results.

We accept most major insurances. Please inquire with our Billing Department for more information about accepted insurances. For information regarding your covered services and benefits, we suggest you contact your insurance carrier. Any co-pay or deductible that your insurance plan requires you to pay is due at the time of services.
